<20> 1963, CIA pilot Ken Collins was flying an inertial navigation <20>
<20> system test mission. After entering clouds, frozen water fouled <20>
<20> the pitot-static boom and prevented correct information from reach- <20>
<20> ing the standby flight instruments and the Triple Display Indicator.<2E>
<20> The aircraft subsequently entered a stall and control was lost com- <20>
<20> pletely followed by the onset of an inverted flat spin. The pilot <20>
<20> ejected safely. The wreckage was recovered in two days and persons <20>
<20> at the scene were indentified and requested to sign secrecy <20>
<20> agreements. A cover story for the press described the accident as <20>
<20> occurring to an F-105, and is still listed in this way on official <20>

<20> caused by a failing fuel gauge, the aircraft ran out of fuel only <20>
<20> minutes before landing. CIA pilot Walter Ray was forced to eject. <20>
<20> Unfortunately the ejection seat man-seat separation sequence <20>
<20> malfunctioned, and Ray was killed on impact with the ground, <20>

<20> This aircraft was lost on 28 December 1967 seven seconds into an <20>
<20> FCF (Functional Check Flight) from Groom Lake performed by CIA <20>
<20> pilot Mel Vojvodich. The SAS (Stability Augmentation System) had <20>
<20> been incorrectly wired up, and the pilot was unable to control the <20>
<20> aircraft 100 feet above the runway. The pilot ejected safely. A <20>
<20> similar accident occurred when the first production Lockheed F-117 <20>
<20> was flown on 20 April 1982 by Bill Park. It's control system had <20>
<20> been hooked up incorrectly. Bill Park survived the accident but <20>
<20> had injuries serious enough to remove him from flight status. <20>

<20> This aircraft was lost in the South China Sea on 5 June 1968. CIA <20>
<20> pilot Jack Weeks was flying what was to be the last operational <20>
<20> A-12 mission from the overseas A-12 base at Kadena AB, Okinawa. <20>
<20> The loss was due to an inflight emergency, and the pilot did not <20>
<20> survive. Once again the official news release identified the lost <20>
<20> aircraft as an SR-71 and security was maintained. A few days later <20>
<20> the two remaining planes on Okinawa flew to the US and were stored <20>
<20> with the remainder of the OXCART family. <20>

<20> accident at Edwards AFB. Lt Col Ronald J Layton and systems <20>
<20> operator William A Curtis were approaching the traffic pattern <20>
<20> when a fire broke out due to a fuel line fracture caused by metal <20>
<20> fatigue. The flames quickly enveloped the entire aircraft and on <20>
<20> the base leg both crewmembers ejected. '936 was totally destroyed. <20>

<20> 1966 for launching the drone, the drone pitched down and struck <20>
<20> the M-12, breaking it in half. Pilot Bill Park and LCO (Launch <20>
<20> Control Officer) Ray Torick stayed with the plane a short time <20>
<20> before ejecting over the Pacific Ocean. Both made safe ejections, <20>
<20> but Ray Torick drowned before he could be rescued. This terrible <20>
<20> personal and professional loss drove "Kelly" Johnson to cancel the <20>
<20> M-12/D-21 program. <20>
